New Patients

How to Register with the Practice

We have an open list and welcome requests for registration from patients moving into the practice area.  

 

Our practice boundary area for new patients is as follows:

Penrhyndeudraeth, Porthmadog, Blaenau Ffestiniog, Trawsfynydd, Harlech.

The cut-off points for each area direction are:

 

Criccieth direction - Cut off at Plasgwyn Nursing Home

Caernarfon direction - Cut off at top of Penmorfa

Beddgelert direction - Cut off before the Aberglaslyn Bridge

Blaenau Ffestiniog direction - Cut off at the Slate Caverns

Bala direction - Cut off at top of Cwm Prysor

Dolgellau direction - Cut off at end of Bronaber

Barmouth direction - Cut off at start of Talybont at junction of A496 with Ffordd Gors just before St Ddwywe's Church

Registration

Patients will be asked to complete a registration form and questionnaire when registering. You can view and download a copy of the registration form by clicking this link.  Proof of identity and residency will be requested so you will need to bring the completed form into reception in person.

If a patient changes address or phone number, they need to let us know. They should also let the hospital know if they are under its care too.  Phone numbers can be updated via My Health Online (click for link).

Those moving out of our catchment area will be required to register with a practice nearer their home.
